William Cole (Bruce Campbell) is a very affluent C.E.O. of a successful American pharmaceutical company. He travels to Bulgaria in order to diversify the interests of his corporation and gain an advantageous foothold on the country. He brings his wife Jackie (Antoinette Byron), and the two find themselves disgusted by the rampant communism throughout the country, annoyingly citing its need for Americanization. They are boisterous, rude, self-centered, and ethnocentric in their behavior. Soon, they get into a fight over a piece of jewelry, and William catches Jackie having sex with the jeweler Yegor. Jackie then catches William sleeping with a hotel maid, so she knocks him into a coma with a pipe, then kills Yegor. Dr. Ivan Ivanov (Stacy Keach) and his assistant Pavel (Ted Raimi) gather the bodies of William and Yegor, and replace the damaged parts of William’s brain with healthy parts from Yegor’s. Now that William awakes with Yegor’s brain in addition to his own, the two personalities combine thoughts to formulate revenge on the woman who attacked them.
